“What matters to you?” Day is an annual celebration of putting people at the heart of their care or support.
Health and care teams across Scotland have come up with innovative ways to have “What matters to you?” conversations. Including using inspiring artwork, videos and poetry.
These conversations are helping teams understand what’s really important to each individual. It’s giving them the insights they need to provide good person-centred care. For example, having personalised prompts and reminders for each patient.
A great example of this in practice comes from NHS Borders and can be downloaded here.
This is just one example of so many great stories we’ve heard from “What matters to you?” conversations. On “What matters you?” Day, we want to hear more. On the day, and the weeks leading up to it, we’ll be sharing more stories and tips on starting these conversations yourself.
